Declaration of 2% tax payment – form
On this page you can download the form Declaration on Remittance of 2% of Tax, as well as detailed instructions on how to fill it out correctly.
Along with this declaration, you must also submit to the relevant Tax Office the Confirmation of Tax Payment issued by your employer. Both forms must be submitted by the specified deadline.
